You may also have a bum thermostat, or cr*p (read: block seal, coolant
'stop-leak') on the inside of the heater core. Finally, One of your
blend-doors (in the heat/vent) may not be functioning properly , and
permitting unheated air to bypass the heater core. This could be specific
to the blend door (like its servo or link has failed) or it may be the
vacuum routing switch in the heat distribution lever on the dash (failing
to activate the servo).
I've had the same condition on a Jetta and a Golf. In both cases the link
between the servo and the blend door failed, keeping my teeth chattering.

My crew cab heats great but then it never gets that cold here.
It's possible there is a factory restrictor in line with the core. Check
the Bently. I recall seeing something to that affect.
