this is low for manifold vacuum at idle you dont mention the compression
test but you said it ran great cold so try a compression test cold and
hot
have the heads been replaced the low vacuum could be valve problems or
valves not closing hot I have seen the low manifold readings happen when
the hydraulic lifters over pumping after the engine warms up
