Lift the driver's side floormat and carpet.  BTW, it has an overflow, so if you
see stuff coming out under the van, don't panic; just stop filling.  BTW, once
you're out of danger of freezing, try adding a little vinegar to regular
water...it'll remove hard water build-up.

One of the list vendors, probably...Bus Depot or Vanagain perhaps.  Add .com to
those names for their web sites.

The new ones just press on...very simple.  I got mine from the Bus Depot.  The
bulb side goes on the inside.
