Here is the answer as Z wrote it:

Are you referring to the outside handle of the inside handle??? The inside handle is held on by two bolts (hope the hinge is not cracking causing the looseness). The outside is held on by one screw, generally phillips head, that you can easily see in the door jam. You may need to push the door seal a little out of the way to get to it. But you do not need to remove it. Simply push it back as you go in with the screwdriver. Its the only screw there, just follow the handle straight out to the edge of the door and you will see it.

As far as the speakers........ The are Pioneer 6.5" with an additional tweeter. The diver side is mounted right below the map pocket and the passenger is just above the kick panel. I am very please with the results. I examined the set up this morning with someone who I consider a stereo expert. He, as well, was pleased with the sound...........But I need more. After thinking it through I will be utilizing the panel under the rear seat for 4 additional speakers. I will update the list eventually on the results and photos.
