Could it be that your throttle position switch is sticking when cold and not
closing at idle until the engine warms up a bit?  Check it out.

By the way, something is wonky if you have to unplug your oxygen sensor to
get the van to run correctly.  Either the sender is bad or you have some
other problem.  Just my 2 cents.
