Mark, the POL spud on the regulator has a so-called "excess-flow" device (if not it's highly illegal) that shuts off if the flow is too high. That's one reason why using a BBQ-type regulator is so bad, because it's unlikely that the POL fitting will be correct. So anything downstream of that fitting that causes uncontrolled venting will be quickly shut off. If the valve gets knocked off the tank, that's a different story entirely, but just having the valve open has been covered by the approval authorities.
