If you're giving it gas to start it, you're screwing up the fuel injection system, and you'll likely have problems within a few miles. You might check the adjustment on the EGR to throttle rod. As I remember, starting with a running, warmed up engine, you tighten the rod until the idle speed drops, then back off the adjustment 1 1/16 turns, then tighten the nuts.
