Leaving aside the external fans which attempt to maximize the ventilation to the fins in back (which is truly marginal, and made worse because it's recirculating the inside air instead of taking outside air and passing it through) -- adding a very small fan *inside* the box will do much to even out the cooling there. FridgeMate makes one that runs a month on two D batts, likewise one that wires in permanently and has a switch. Some folks use a tiny CPU-cooler-type box fan, but I'm pretty happy with the Fridgemate one, which is almost silent and needs no permanent wiring (and I put it in Scamp's icebox when I'm sailing instead of rolling).
