When I re-installed the starter in my '85 crew cab after a clutch job, I was able to acccess the nut by sneaking a 17mm wrench up from the lower side. The entry point is about where the exhaust pipe flange bolts to the cylinder head.

I had considered removing the throttle until I noticed that I could get to it from the bottom.
