Ken,

If an injector(s) is (are) leaking that fuel from the pressure in the system at shutdown would cause a flooded cyclinder too.

As to the injection pulse at start up flooding the engine, I would think that the ECU map has a base pulse lenth for the start-up sequence, with perhaps the TEMP I , air temp sensor, giving some modification of the cold start enrichment of the basic pulse map. If the temp I were bad then perhaps a flooding event could occur.
