Works great when it's greased and not rusted solid. Not much to it, the adjuster itself has a ratchet wheel on the shaft, and applying the brake moves a claw over the wheel. When the travel gets long enough the claw engages the next tooth on the wheel, and pulls it back when the brakes are released.
