Have you pulled out the pedal assembly and taken a look at it? I pulled out mine when I replaced my master cylinder and found that the hole in the pedal where the clevis/plunger connects had elongated to almost twice its size. This would definitely change the angle the plunger would enter the clutch master cylinder at and possibly cause premature failure.
