My radiator fan was making a lot of noise on high speed last summer. I removed it, disassembled the motor and lubricated the bearings. You could bring it to an electric motor shop to have them rebuild it.

Also, it is possible to lower the condenser and radiator enough to remove the fan and shrouding without disconnecting any hoses. I didn't even jack up the van, but I did use my hydraulic jack to lower the radiator/condenser/fan assembly since it is quite heavy.

As far as I know, the radiator fan is only two speed. The high speed circuit is controlled by the temp switch on the radiator. The low speed is controlled by the A/C compressor and the radiator temp switch. The A/C should switch on the low speed fan regardless of water temperature. R12 level shouldn't directly influence the operation of the radiator fan, only the operation of the compressor and A/C system.
