Kim, Could be a number of things... I suspect you have a small leak from compression into your cooling system. The place to vent this is in the front at the radiator. Seems air bubbles collect at the top of the radiator. The air then pushes the coolant out the back of the system.

Now with your discovery of a loose fitting going to the rear heater, perhaps that is where you are drawing (or were drawing) air in. Try bleeding the system again and see if the problem goes away. Think about replacing that 'T' before it separates and cause even bigger problems.
