It's a standard fitment on European models, I was devastated when I moved out to the US to find that they were all four speeds.

I'm told there is company over here that does a 5-speed conversion, Kennedy Engineering in California, though the price is about $3500. Alternatively you could see about importing one. You'll need to check the ratio's on your final drive as well though otherwise you'll find it overgeared in the wrong direction, some of the four speed waterboxers use the same final drive and some don't.
