After replacing everything that would be included in an extensive tune-up, and fixing the AFM, I finally solved this problem by replacing every vacuum line, seal and gasket I could reasonable do in an afternoon.

Van is hesitating from a full stop. Apply WOT and nothing after a little bit it will start to go but hesitantly. Have applied air meter fix, could it be that the rails are too worn to send a signal to give gas to motor? Could it be another sensor? O2 just recently replaced. Give me some ideas of what to check and where to check them :o) Thanks for the help and will keep updated..
