I removed the temp sensor and the van ran wicked rich. I believe the resistance of the temp sender drops from around 3000 cold to around 300 ohms when the engine is at temp. I checked it while cold and read 3K ohms. I'll have to go back and take another look at it.

The O2 voltages I measured were made with the O2 sensor disconnected causing the FI to stay open loop.

Remove the coolant temp sensor for the ECU - does the idle smooth out? Seeing how you replaced the AFM and still have the problems this is one of the other "influential" items for the ECU - better yet, get a 15K Ohm resistor and install this across the terminals of the coolant temp sensor - this will make the ECU think the engine coolant is warm and everything is fine.

The O2 sensor should be 0.7V and fluctuating up and down 0.2V or so - this could be the source of the problem, however, unplug the O2 sensor and see if the idle smoothes out.
