Could you explain that a bit further... I understand the loss of temper and everything but with a cotter pin in it so I can't turn. Is the nut being crushed? IMHO....(very humble) I would almost be inclined to say that he should raise the wheel before tightening and use the tool with the hand sledge and breaker bar. On the other hand he could be having a heat problem in the axle and the heat is heat stress annealing the axle from the stretch the nut is putting on it. so I'd repack the bearing too.

It spoils the heat treatment and makes the metal soft.
