I'm not saying this is what's happening (though it wouldn't surprise me). But that nut is torqued very tight, and if the nut -- the special 10-slot nut that they put on when they increased the torque spec by 50 per cent or so -- has been softened, I could easily imagine it deforming the threads and loosening under load.

Also, once the hub has worn so that there's no longer a flat contact between the nut and the hub, all bets are off until both surfaces are flat again.
