Yes it can be done with the engine in the vehicle. Install 6" long bolts in the mustache bar mounts at each end and drop the engine down in the rear. It gives you much more room to work. Pulling your pistons has to go in a special rotation to clear the oil cooler mount and there are just a few other tricks. However having done it that way I can tell you it doesn't save any time or labor in the long run.
