You do not mention the situation with your hydraulics - is there any leakage at the master or slave cylinders? Any fluid missing? Are you *sure* that pumping really works?

The reason I ask is that I experienced something very similar recently and it turned out to be a combination of things, but primarily a failed pilot bushing. Since we had to pull the trans to replace the pilot bushing, I had it sent to Weddle for a "check-up". Well, needless to say I now have a rebuilt transmission (thanks Chris) and I was astounded that the thing shifted at all based on the condition of the old pieces. The housing for G-gear/reverse was totally cracked and a couple of bushing areas were ovalized.

While I had the trans out I went ahead and put in a new Bosch starter to boot, since it is such a PITA to get to normally, and it had about 70k on it.
