Place it where in a spot similar to the handle in the passenger side. This is where the handle is installed in the RHD vehicles.

See above answer. Be careful drilling the mounting holes though. There are wires running up the A-pillar which you can easily short out by carelessly drilling. Thanks to Jeff (AKA Oxroad) for helping me avoid that pitfall.

Doesn't seem to be any different from yanking on it when you are driving. It is designed for those kind of pressures I would think. That is unless you have a buffalo butt like my buddy Mike Miller

Depends on which one you use. Best way to install is by pop-rivets. Sheet metal screws are OK too but now as secure. Not to mention makes them removable by vandals or thieves.
