If your engine is smoking, it's overloaded. Common problem with sailboat diesels, where the builder puts on too big a prop so the motor can't attain full rated rpm. I think that WOT with steady/falling rpm in an automotive situation would have the same result with an engine where the throttle directly controls the injector rack. Using more throttle than the engine can accelerate with isn't pussyfooting, it's overworking.

Don't have automotive sources, but there was an article in Cruising World magazine 5-10 years ago that went into the problem in some detail (from a marine perspective of course).
