Not rare at all. I see it here all the time and it's becoming more common as these Vanagons get older. There is a spacer between the inner and outer bearings. The spacer is worn and not the correct length. Replace it. Replace the bearings also if they are worn. The axle threads probably got stripped while the nut was loose and you were driving the vehicle with the loose nut. You will probably need a new or used axle and nut. I would put new bearings in there also as it's a real PITA to take it apart again. I would also highly recommend you do the other side just to be prudent and have worry free driving if that's possible with a Vanagon !
