I had the same problem, but it got so bad the van would just 'pop' out of reverse. One lower bolt was broken off, the other just loose. I tightened the loose one, and all has been well since. Ive yet to fix the broken one (yikes).

My slave had a slow leak, and was difficult to shift into first. I replaced the slave, and viola, smooth as butter baby.
