I have the same problem. The shaft is pushed out by the white worm gear. I attached a spring on the end to keep a little bit of pressure on the shaft. Malcolm did the same fix with a piece of plastic.

I do not think there ever was a nut on the end. The white worm gear or other part must be worn out which causes the shaft to be pushed out.
