It depends on the car, I think. A lot of the modern Japanese cars they're selling these things for actually will compensate automatically if you have a good oxygen sensor -- the ECU has enough range in its fuel trim to supply the extra fuel when it sees it's running lean. I imagine you'd have to be careful, though, seems like you could break things in a hurry if the O2 sensor ever failed.
