Entire plastic lever slips off the metal lever -- warm the plastic beforehand and pull on the shaft, not the knob. If the knob comes off by itself I don't think it's practical to replace it, there's not enough bearing surface there for glues etc.

Beware coming off -- the edge of the internal socket likes to catch on the edge of the faceplate slot and break it.

If it comes off very stiff, slit the upper surface between the detent hole and the end before replacing.
