See if the exhaust system is striking something when the engine first fires...this *might -- I can't remember for sure* be more likely if the engine carrier (transverse tubing bolted to frame rails and holding motor mounts) is installed backwards -- mine was. Unfortunately I can't tell you how to tell without removing the thing -- it has directional arrows unobtrusively forged into the mating surfaces where it bolts to the van. There's a measurement fore-and-aft from somewhere that will tell you, but I can't say offhand what it is.
