Check your front trans mount - this can cause problems similar to this. Also, rotate your tires to see if the problem changes. Or, at the very least, spin each tire on the van with it jacked up, and look for lumps in the tread. A broken belt can also cause vibration, but should be very noticeable if moved to the front. It should also have been caught during balancing, but not always...
