I read this on a UK Vanagon site: One clever way of removing the bush is to find a metal rod whose diameter is just smaller than the inside diameter of the bush. Put a large blob of grease on the end of the rod and insert the rod it into the bush and push. The grease will force out the bush.

I haven't tried this myself, but at least two people said they had, and that it worked for them. (I was planning to replace my starter because it turned the engine very slowly, but replacing the gearbox ground strap - and cleaning the metal where it connected - cured the problem.)
