Two things come to mind:
1) If the outer sheath of the coaxial wire that carries the O2 sensor signal
to the ECU shorts to the inner wire - something that seems to happen where
the wire attaches to the ECU plug - then you'll always experience rich
running.
2) If you have an AFM then over time the spring that resists the twist of
the air vane can weaken, leading to rich running at all speeds and in all
conditions.
