I once saw an MG that had been modified for a guy who had a very bad left leg. A hand lever, like a motorcycle clutch lever, had been mounted on the shifter and connected to a clutch slave cylinder. Might be something to think about if your client has good hand strength. It'd be a pretty easy mod compared to full solenoid-controlled shifting.
