Not having first-hand experience with the Vanagon hinges, I can only
offer what I observed when working in an auto body shop.  Several cars
have started to have door hinges attached with adhesive. I recall on many
occasions watching in wonder as my boss literally grabbing the door and
lifting/twisting/manipulating as necessary to get a good fit.  Granted,
he was quite a large and burly fellow.  As BenT suggested, a well placed
jack would probably be the next best thing.
