Warren,

It seems like you have touched upon just about all the common causes. Have you checked or replaced the hall sensor in the distributor yet? I had a problem with mine a while ago and it drove me crazy because it was intermittent. These tend to break. I would suggest swapping the whole distributor if you have the option. Be careful not to put any stress on the hall sender and wiring if you replace it.
