Doug,

Jack, front - Under the control arm as close to the wheel as possible; a softener (small piece of wood) between the jack and the point of contact on the suspension member is a nice touch.

Jack, rear - Under the trailing arm directly beneath the axle and as close to the wheel as possible.

Stands - Under the jacking point receivers.

Unless you have some experience with rigging, it is most prudent to lift the van on a hard, level surface. Once the car is elevated, push on it hard in all directions to make certain that it is well planted and has NO tendency to roll, tip or slide off of its supports. Before I climb under a lifted car, I have the habit of placing some manner of redundant support which is tight to the car, but not taking any load, in a central location. A shadetree mechanic has do his own safety engineering. Be careful.
