An inline 4 cyl engine needs to be either a 90 degree or 180 degree crank. With a 180 crank, 2 pistons move up and down exactly together while the other 2 pistons move exactly together 180 crank degrees out of phase with the first 2. There appears to be slightly more bore area showing on the left end than the right end though it may be a distortion in the photo.
