probably the cheapest US wide solution at the
moment is T-Mobile.  Get a phone with them
(screaming deals on Amazon.. esp for the sidekick II
now if you are a new subscriber.. wish they would do
the same for their current customers :-(   )

but anyways..  using the phone as a bluetooth
modem their GPRS network gives you 40Kb/s
and usually works whever you can get a cell
phone signal.   The other thing you can do is
then bundle the T-Mobile wi-fi access and
hook up at any Starschmucks, Kinkos or
B&N/Borders? (cant recall) bookstore that has
a Starschmucks in it.  I think the rate is $19.99
per month if you have a voice account with them.

That still puts you at  $50 a month (probably more
like $60 when you add taxes and other fees)

I dont know if there is any other provider yet that
is offering such for that amount.. (and has such
ubiquitous access)  ?

I use a combination of the above.. along with
the Sierra Wireless Card (T-Mo again) but it's
all paid for by my company and I dont see the
bill the latter
