There is an article on the Volkscafe website about CV joint maintenance and diagnosis that should answer the questions about which noises mean failed CV joints and how to repair them.

Included in the article is a diagram that shows how to rotate the joints to alter the pattern of wear in what I interpret to be the same manner described earlier by Dennis Haynes. Also has a diagram of what will not alter the pattern of wear.
