There are three headlight adjustment pieces per headlight (12 in all). On each headlight, one is a fixed length and the others are adjustable to align the headlight (one adjuster for up/down and one for right/left). The adjusters consist of a metal rod with plastic fittings on each end. The plastic gets brittle with age and breaks, which allows the headlight to flop around.

To fix, you need to replace the broken adjusters. I did this recently, and I can tell you from experience that you should replace ALL of the adjuster pieces at the same time - because the other ones are just as brittle as the broken ones and they will break when you try to replace the ones that are broken now...

The headlight adjustment pieces can be purchased from the various list venders - but they aren't that cheap. But then, they DO last for 10-15 years, so...

My buddy's van has the rectangular headlights and I think there is some hardware missing. They swing loosely as if they are hinged at the top but not connected at the bottom. I'm not familiar with these as I have round ones. Does anyone know what he's missing?
