In desperate cases I put a glob of weld on the lever on either side of the shaft hole and then use a small pitman arm puller on the lever. The globs provide something for the puller arms to grab. I had to laugh recently when someone suggested pulling the lever off before pulling the tranny to keep from having to mess with the clutch slave. I'm sure that will work sometimes but the efforts to remove the lever can prove to be much harder than dealing with the slave. The clutch cross shaft that the lever goes on runs in plastic sleeve bushings inside the bell housing so it is easy to damage those parts while messing with the lever.
