Remove the unit from the tank and clean it. Be sure there are no cracks in the plastic anywhere especially between the electrodes. Also check your coolant for the correct antifreeze content. These vans are set up for a 50/50 mix of water and coolant. If not correct you will likely get an erroneous reading and the light will come on. If the coolant has not been changed in the last two years the system needs to be drained, flushed and new coolant installed.
