You got a problem all right!

Could be nothing more than a hole in the line from the coolant tank over to the expansion reservoir or a bad pressure cap.

I would pressure test the cooling system and see what happens. A pressure cap is cheap enough, and the hose is even cheaper. So even if it's neither of these, it's cheap enough just to eliminate them from the pursuit of the problem.
