This all would be a good indication of whats going on.

I can't really say as I'm not the original owner, I can say that in the 4 plus years that I've owned it I haven't done that. The coolant was changed almost two years ago when I put the new to me used engine in it. The PO put a new radiator in the van back in 1993. The van has 184,500 miles on it. I'm wondering if I didn't get the spring clamp on correctly. It's in an awkward spot and maybe I didn't get it up high enough on the fitting. I think I'll try it again and see what happens. If it blows off again then I would assume that the radiator needs to be boiled out or replaced. What do you think?
