I think you have a bad hose or the wrong hose. The spring clamp may also
be weal from age. They do fatigue and they should be replaced with the
hoses or radiator. The water can not make enough pressure to blow off a
properly secured hose although total system pressure can. Make sure the
pressure relief cap is working and hope that you are not pumping
combustion gasses into the cooling system.

I can't really say as I'm not the original owner, I can say that in the
4 plus years that I've owned it I haven't done that. The coolant was
changed almost two years ago when I put the new to me used engine in it.
The PO put a new radiator in the van back in 1993. The van has 184,500
miles on it. I'm wondering if I didn't get the spring clamp on
correctly. It's in an awkward spot and maybe I didn't get it up high
enough on the fitting. I think I'll try it again and see what happens.
If it blows off again then I would assume that the radiator needs to be
boiled out or replaced. What do you think?
