What did you set the initial gap at the throttle stop at?

When you adjusted the TPS switch what was the gap between the stop screw and the stop?

How worn is the cam?

What is the multimeter readings open and closed?

If this isn't all right on the money, the switch cannot be set properly which will cause all the problems you described.
