The only thing I have seen effect all gears as you are mentioning is if the clutch/throw out bearing was not functioning correctly. This could be the master/slave/bearing/shift arm that controls the bearing, and pressure plate. If it is anything internal it usually lets you know very loudly and clearly that it is internal and not just at shifting.
