The eventual fix was replacing every part (new or used) that connects to or
talks to the engine. Not a good way to go if you are squeemish, but I got
tired of the individual bills and paying labor to throw parts at it. I was
initially trying to diagnose it by doing part by part, but after several
months, bought all the used parts I could and replaced them all at once.
New engine, new hoses, new coil, new cap, rotor, plugs, new plug wires, used
ICV, used IC relay, used AFM, used ECU, used fuel pressure regulator, used
fuel pump, new filter, probably forgetting something. Screw it, it runs
good now.
So 175 from a dealer that spent 5 days on it seemed like a bargain.
