Not an engineer but my understanding is that it's an engine wherein a loss of timing between the crankshaft and valve train (such as would occur if a timing belt slipped or broke) would result in Mr Piston getting to meet Mr Valve with typically diastrous and expensive results. I think some engines (my old Fiat was one) could also have interference between the crankshaft and the fuel pump lobe under those same circumstances. Geo Hahn
