Check the T2 sensor and the wires and connector for it, I had a similar problem once and the connector had come undone.

Mine was also hard to start once warm. Chatted with my mechanic about that today. He asked "was I depressing the gas pedal at all?" I said no. He said "try it. Once it's warm the cold start circuit is off. You need some airflow for the AFM to signal the injectors to fire." Works like a charm now. Starts on the first try hot or cold.
